Ontario Public Health PractionersIt was planned from the start that Mujer Sana/Comunidad Sana would transfer aspects of its work to other communities and seek long-term funding. The project team knew the 2.5-year timeframe was too short to show whether the project had long-term effects, such as whether women would continue to be screened for cervical and breast cancer. So short-, medium- and long-term outcomes for individual, organizational, and community change were developed and included in the project logic model. The project team also developed sustainability indicators:

  1. Key organizations continue to work together on joint programs after the project is finished.
  2. Lay health promoter positions continue at health service delivery organizations.
